Alliance Health Care

Our Mission is to maintain a progressive, nurturing environment with a team dedicated to fulfilling the highest standards of quality care while encouraging consumer independence. A member of the Personal Care Provider Organization (PCPO) with Unlicensed Traditional Personal Care Assistance Services (PCA) and PCA Choice options providing Qualified Professional Services and Supervision.

Alliance Medical Supply

Providing pediatric and adult medical equipment & supplies including:

incontinence & urological supplies, nutritional supplements, hospital beds, wheelchairs, patient lifts folding & four wheeled walkers, bath safety equipment and breast pumps.

Accepting Minnesota Medical Assistance, county waivers, Medicare, private insurance providers and private pay.

Alliance Fiscal Support

State certified Fiscal Support Entity providing support to individuals utilizing participant directed services through Consumer Directed Community Supports (CDCS) and Consumer Support Grant (CSG) programs. Service models available include Agency With Choice, Payroll Model (Fiscal Employer Agent), and Fiscal Conduit.

Alliance Health Services

Providing Comprehensive Home Health Care with Licensed home care services including skilled nursing, home health aide, homemaking, companionship.

245D Home and Community Based Services – Licensed waiver services including In Home Family Support, Independent Living Services, Community Residential Services, Personal Support and Respite.

Case Management Aide and Mental Health Services including Adult Rehabilitative Mental Health Services, In Home Therapy, and Group Therapy.
