Individualized Home Supports (IHS) are designed for individuals living in their own or family homes who require support or training in community living services. There are three IHS categories:
Eligibility: Must live in their own home and be on a waiver.
Basic: Providing support for daily activities and community engagement.
Intensive: Offering training to increase independence and achieve specific goals.
Family Training: Assisting families in supporting individuals within their homes.
Night Supervision provides overnight assistance and supervision for individuals living in their own homes when assessed as necessary.
Eligibility: Determined through a county assessment.
Respite Care offers short-term care services for individuals when their primary caregivers are absent or need temporary relief. Contact your Case Manager to get more information on your fit for this program.

Homemaking Services facilitate the daily upkeep of the home in the Twin Cities metro, Sauk Rapids, Saint Cloud and Mankato areas. These trained professionals can provide the following services:
