CFSS is a new self-directed service (replacing PCA and CSG) that offers flexible options to support you at home and in the community. Alliance offers CFSS under both service delivery models (Agency Provider Model and Budget Model).
Alliance will help you understand and access your CFSS service options and supports. We provide education, assist with writing your service delivery plan, if desired, and offer ongoing support with your CFSS services.
Alliance helps you manage your self-directed services and budget so you can enjoy the freedom to customize your personal care and supports as you see fit. FMS works with individuals through CDCS (Consumer Directed Community Support), CSG (Consumer Support Grant), and Budget Model for CFSS (Community First Services and Supports)
A variety of mental health services to help you achieve a productive and satisfying life. Options include therapy for individuals of all ages, groups, couples, and families, as well as services for those on medical assistance including ARMHS (adults), peer support, family peer support, and CTSS (children).
Qualified care to support your daily living skills and/or complex health care needs in the comfort of your own home. Options include Skilled Nursing, Home Health Aide Services, Homemaking Services, and Personal Care Assistance (PCA) Services.
Home and community based services to meet your individual support needs, preferences, and goals. Options include Individualized Home Supports (IHS) - basic and intensive, Night Supervision, Respite Care, and Specialist Services.
Durable medical equipment (DME) and supplies to support your individual medical needs. Nutrition, incontinence supplies, assistive and safety equipment for bathroom/home, support garments, mobility supports, etc.
A Legacy of Care
In 1989, a parent saw that the needs of her child with Down Syndrome stretched beyond what her family could provide.
And while she knew how to navigate the healthcare system to get the home care her child needed, she knew others didn’t. That’s why she started Alliance — to help bridge people and their loved ones to the care and support they need.
Alliance started in 1990 as a Personal Care Agency and has grown to become a multi-faceted service provider offering a diverse menu of services and supports to people in the disability, aging, and mental health communities. We know it’s about more than just getting the health care services you need. It’s about trusting your providers and caregivers to honor you and your choices. You deserve to live a full and supported life that is so completely YOU!
