Where do we provide services?

We provide services throughout the entire state of Minnesota. Please contact us directly to see if we have coverage in your county or region.

How do I get started with services?

The best way to get started is to submit a referral form that provides us with background information about your service needs and personal preferences. You can also contact us here to speak to us about services options for you and/or your loved ones.

Can I have multiple services with Alliance Health Care?

Yes! That’s one of the best parts of working with Alliance. You can have most of your service and support needs taken care of in one place. If there is something we can't directly help you with, we will provide you with resources. 

Do I need insurance?

We accept most major insurances and we do accept private pay for some services. When you submit a referral form, we can review your information and give you specifics on payment options for your needs.

How do I get a copy of my medical records?

You have three ways you can do that: 

  1. Send us a contact request.
  2. Call us at 651-895-8030
  3. Fax us at 651-895-8070.
How do I submit an order for medical supply?

New clients: Submit a referral form.

Current clients: Contact us.

What items aren’t available from Alliance Medical Supply?

Alliance Medical Supply does not currently offer CPAP equipment and supplies, Diabetic equipment and supplies, hospital beds and accessories, wheelchair and accessories, and Motorized Mobility Equipment.

What are common abbreviations and definitions?
  • CDCS = Consumer-Directed Community Support
  • ARMHS: Adult Rehabilitation Mental Health Services
  • CTSS: Children Therapeutic Support Services
  • CSG = Consumer Support Grant
  • DHS = Department of Human Services (Minnesota State Department)
  • CFSS= Community First Services and Supports
  • FMS = Financial Management Services 
  • Participant/Client/Recipient = the person receiving services
  • PCA = Personal Care Assistance
  • RN/LPN = Registered nurse/ Licensed practical nurse
  • RP/Responsible Party or PR/Participant Representative = the person designated to make decisions on behalf of the client, if the client is not able to direct their own care.
  • QP = Qualified Professional
